Semaglutide is the newest GL P-1R agonist and was approved by the FDA as a once-weekly injection for the treatment of T2DM in late 2017. The peptide is chemically similar to liraglutide, with the inclusion of two structural modifications (Lau, Bloch, et al., 2015). The first is the replacement of Gly with the non-proteinogenic amino acid 2-aminoisobutyric acid (Aib) at position 2. The second is the attachment of octadecanoic diacid to the side chain of Lys-26 through a short polyethylene glycol (PEG) spacer and a γ-glutamic acid linker. The amino acid substitution renders the peptide more resistant to DPP-4 compared to liraglutide, and the presence of the 18-carbon fatty acid moiety results in a high binding affinity for serum albumin, which translates to a half-life of approximately 7 days in humans

Tirzepati de is a synthetic linear peptide molecule containing 39 amino acids. Residues derive from G LP-1, GIP and sema glutide, and a few residues are unique [30]. More specifically, the structure is based on the native GIP sequence and includes C20 fatty diacid moiety (eicosanedioic acid) linked via hydrophilic linkers (γ-Glu-2xAdo, gamma glutamate and bis-aminodiethoxyacetyl) connected to lysine residue at C20 position [31]. The peptide sequence of tirz epatide contains two non-coded amino acid residues (Aib, α-amino isobutyri acid) at position 2 and 13, which are responsible for its long half-life and high affinity to albumin [32]. The C-terminus of the peptide is amidated (Figure 2) [33]. The molecular formula of tirzepati de is C225H348N48O68 and the molecular weight is 4813.45. Tirze patide is the first agent that functions as a dual agonist for the two main human GLP -1 and GIP incretins, a promising drug against both T2D and obesity [34,35,36,37]. It has impressive glycemic efficacy. Moreover, it is the first effective drug to have demonstrated notable body weight loss in a phase 3 study in patients with T2D.
